Event Participation Agreement
Please fill out and send  this agreement to Rick Stomps seven (7) days prior to the meet.  ( 810-631-9826 - fax)

Parental Consent Form
Each clubber will need a new form filled out for each event in which they participate.  Clubbers without a form signed by their parent or legal guardian will not be allowed to participate. If the form is altered or amended in any way it will be invalid.

Day Event Volunteer Form
Every person who volunteers at one of the regional Awana events (Grand Prix, Awana Games, Sparks-a-Rama, and Bible Quizzing) MUST submit the form. It may be faxed to Rick Stomps at  810-631-9826
