IMPORTANT FORMS:

   EVENT PARTICIPATION AGREEMENT
Each church needs to have one Event Participation Agreement on file.

PARENTAL CONSENT FORM
Each clubber MUST have a signed form for EACH Awana event.

   DAY EVENT VOLUNTEER FORM
Every person volunteering at an event (judges, starters, scorekeepers, etc.) MUST have one of these forms.   It may be faxed to Rick Stomps at 810-631-9826.
