$25 per team if postmarked by February 1
$30 per team if postmarked after February 1

ORGANIZATION OF TEAMS
Teams = 1 - 3 members (2 quizzers plus 1 alternate)
Book 1 Grades 3-4 ... Co-Ed or all same gender
Book 2 Grade 4 ... Co-Ed or all same gender
Grades 4-6 ... Co-Ed or all same gender
Book 3 Grades 5-6 ... Co-Ed or all same gender
Book 4 Grade 6 ... Co-Ed or all same gender
New 5th 6th grade Discovery Books
Book 1 Grades 5-6 ... Co-Ed or all same gender
Book 2 Grade 6 ... Co-Ed or all same gender

Each clubber MUST have a signed Parental Consent & Release of Liability. A new form is required for each event. No one can participate without a completed form.
